What You'll Do
The Journey Health Community Health Worker (CHW) provides care and administrative services as part of a multidisciplinary care team under the supervision of a clinical practitioner. You will also be a key member in removing barriers that prevent patients from accessing needed care and in supporting them to achieve their care plan goals.

Responsibilities

Administer health screenings, diagnostic testing, and treatment procedures under the supervision of a clinical practitioner
Support patient care delivery by preparing laboratory specimens, performing basic laboratory tests, and conducting select clinical procedures to include but is not limited to the administration of medication and vaccinations/injections under the supervision of a clinical practitioner
Assist with other clinical and administrative duties to ensure optimization of the patient experience and management of their care
Conduct telephonic and/or face-to-face outreach with patients to identify social determinants of health impacting patient's health and overall wellness
Coordinate with the patient, caregivers, and other members of the care team as necessary to ensure the care plan is implemented based on patient-directed priorities and goals
Utilize coaching, motivational interviewing, and other evidence-based techniques to support patients in achieving their goals
Identify barriers to achieving targeted clinical or social outcomes, and engage the care team to revise the care plan when necessary

What You Need

Qualifications

  • Community Health Worker Certification, or has 2,000 hours working as a CHW (or similar role) in a paid or volunteer position within the previous 3 years and is committed to becoming certified within 1 year of hire

  • Experience navigating the healthcare system (this helps with the idea of hiring from the community)

  • Certification in basic life support (BLS)Demonstrates experience in providing culturally sensitive care and delivering high-quality customer service to diverse communities.Communicates clearly and effectively with team members, clients, and stakeholders.

  • Work independently, make informed decisions, and manage tasks efficiently in a fast-paced environment.

  • Support colleagues, cross-train on diverse job functions, and foster a cohesive, high-performing team.

  • Embrace organizational changes in a dynamic, start-up environment while remaining adaptable and proactive.

  • Proficiency in Microsoft Suite applications (Word, Excel, PowerPoint, Outlook, and Teams)Familiarity with motivational interviewing, clinical documentation, and the healthcare system
