What You'll Do

The Journey Health Manager of Social Services develops, implements, and evaluates the delivery of social services as part of a multidisciplinary care team. You will manage a team of Community Health Workers to ensure patients’ social and clinical needs are met for successful long-term health maintenance. You will provide value-added services that are linked to strategic organizational goals and objectives.

• Plan, implement, maintain, and improve the delivery of social services provided as part of a multidisciplinary care team
• Develop, execute, and monitor the quality management programs for social services
• Manage, train, and ensure the competencies of Community Health Workers
• Identify and collaborate with community organizations to enhance community services for members
• Provide clinical leadership and supervision/consultation
• Work closely with the clinical team to help them process the emotional and psychosocially complex
nature of the work
• Collaborate with Journey Health’s care management team which includes physicians, nurses,
pharmacists, therapists, social workers, community health workers, and medical assistants
• Ensure compliance with federal, local, and state regulatory standards
• Perform other related activities as assigned

What You Need

  • Master's degree in social work from an academic institution accredited by the Council of Social Work Education

  • Current LCSW in the State of California

  • Minimum 3 years of clinical social work experience, including at least 2 year of experience in social work supervision or program management

  • Certification in basic life support (BLS)

  • Skilled in delivering a holistic view of health for the whole person that considers physical, behavioral, and social drivers

  • Possesses the knowledge and skills to perform comprehensive social work assessment and develop actionable health plans

  • Effective communication skills and experience working with other clinical providers, patients, families, and community resource organizations
