What You'll Do

The Journey Health Nurse Practitioner will deliver in-home medical care to patients as part of a multidisciplinary care team. You will work with patients and caregivers in the home to coordinate care, educate on medical needs, and ensure patients’ social and clinical support for successful long-term health maintenance.

Responsibilities
• Conduct telephonic and/or face-to-face outreach with patients to identify social determinants of health impacting patient's health and overall wellness
• Physically examine patients in their home
• Review diagnostic tests
• Reconcile medications
• Create chronic condition management care plans
• Provide transitional care education
• Collaborate with patients around their health care goals
• Partner with Journey Health’s enhanced care management team which includes physicians, nurses,
pharmacists, therapists, social workers, community health workers, and medical assistants
• Perform other related activities as assigned

What You Need

Qualifications

  • An advanced practice nursing degree from an accredited college or university

  • Nurse practitioner license in the State of California

  • Experience as a Nurse Practitioner/Registered Nurse in a clinic, hospital, ER, or other healthcare delivery setting

  • Certification in basic life support (BLS) required

  • Effective communication skills and experience working with other clinical providers, patients, families, and community resource organizations

  • Proficiency in primary care, acute care, and chronic care management practices

  • Knowledge of health and patient care regulations, and familiarity with value-based care delivery models
